DescriptionRegister Nurse
Position Summary
Promotes individual(s) health goals by completing the nursing
process for assessment. Collaborates with physicians and IDT.
Identifies individual care needs, documents care services in
individuals EHR. Maintains safety, asepsis, and ensures the use of
universal precautions are followed . Provides quality care by
adhering to therapeutic standards and measures health outcomes
against individual care goals. Is available in person and remotely
for triage and provides clinical support to unlicensed personnel as
needed. Ensure continuity in care. Must hold an unencumbered
license for NY State issued by the New York State Department of
Education as a registered nurse. Background in home health,
developmental disabilities or mental health preferred.
Key Responsibilities
- Evaluate and identify individual care requirements on a
continuing basis.
- Completes health reports weekly
- Weekly review of health tracking and medical records
- Ensure that universal precautions are implemented and
maintained.
- Administration of medications and treatments
- Weekly review of medication EMARS/TARS to assure that
medications are being transcribed and administered as
prescribed.
- Ensures that prescriptions are renewed and/ or refilled, and
medications orders are recorded accurately.
- Review all medication errors with coordination and
implementation of corrective actions in collaboration with the
Residence Manager/AD.
- Reconciles medications monthly and ensures that outdated or
discontinued medications are disposed of in accordance with
regulations.
- Inspection of medications storage areas for
compliance.
- Ensure, in conjunction with the IDT, that all medical
recommendations are carried out and brought to resolution.
- Meets with Manager/Medical coordinator/IDT team weekly to
ensure that all recommendations and follow ups are communicated and
implemented.
- Confer regularly with medical providers including pharmacy and
nutrition on going as needed.
- Completes and documents all health assessments in accordance
with regulations.
- Creates and updates care plans for all diagnoses, medical ,
adaptive and assistance care and initiates training for the
unlicensed personnel.
- Review and update of diagnosis list and immunizations in the
electronic health records.
- Prepares training materials and schedules continuing education
and in-services -for DSP that is residence and individual care need
specific.
- Certifies AMAP on completion of initial training and annually
thereafter. Identify deficits pertaining to safety with medication
handling and administration.
- Attends and participates in agency meetings as
required.
- Carry an agency cell phone and be available to all AMAP/DSP to
triage.
- Completes other task delegated/assigned by direct
supervisor.
- OSHA guidelines followed pertaining to sharps storage and
disposal.
- Completion of annual self-medication Evaluation.

Pay Rate
$56.00 per hour / Part-Time
Education and Experience Requirements
Bachelor or Associate Degree in Nursing
1-2 years clinical experience.Qualifications
EducationAssociates (preferred)Bachelors (preferred)
ExperienceBSN or better; 1-2 years clinical experience.
